I have a feeling cost plays into a lot of it. Americans have to work harder to earn the money they once did and many are forced to save for their own retirements instead of relying on their employers for pensions. Also employers tend to pay less of the share for health care plans. I personally don't judge them as I don't know what's going on in anyone's personal sphere.
